Introduction : L’ankylose temporo-mandibulaire correspond à la disparition des structures articulaires normales qui sont remplacées par un tissu fibreux et/ou osseux et qui entraine la soudure de la mâchoire au crâne avec comme principale conséquence une limitation souvent serrée de l’ouverture buccale. Chez l’enfant, elle entraîne de plus de graves troubles de la croissance mandibulaire qui est responsable de déformations faciales. C’est dans ce cas une "quasi urgence thérapeutique". Le but de ce travail était de rapporter un cas d’ankylose temporo-mandibulaire bilatérale et les difficultés liées à sa prise en charge.
Observation : L’observation concerne une patiente de 16 ans, élève, en provenance d’un pays voisin, admise au service d’ORL du CHU mère-enfant “Le Luxembourg” de Bamako pour une douleur dentaire avec déformation faciale. L’examen clinique et radiographique ont permis de poser le diagnostic d’une ankylose temporo-mandibulaire bilatérale type III de Topazian. L’intervention chirurgicale a été réalisée sous anesthésie générale entraînant une résection des blocs d’ankylose avec coronoïdectomie bilatérale suivie d’une interposition du muscle temporal. L’ouverture buccale en per opératoire a été de 45 mm. Une cale interdentaire en résine a été placée en fin d’intervention pour maintenir l’amplitude de l’ouverture buccale. Une rééducation post opératoire précoce a été entamée dans les 24 heures. Les suites opératoires ont été simples. La patiente est revue six mois après l’intervention avec une ouverture buccale à 40 mm.
Conclusion : L’ankylose temporo-mandibulaire est une pathologie très invalidante, inesthétique et son incidence reste encore importante dans certains pays en voie de développement.
Introduction: The temporomandibular ankylosis corresponds to the disappearance of normal articular structures which are replaced by fibrous and/or bone tissue and which leads to the jaw to the skull being welded with the main consequence being an often tight limitation of the mouth opening. In children, it also causes severe disorders of mandibular growth that is responsible for facial deformities. It is in this case a "quasi-therapeutic emergency". The aim of this work was to report a case of bilateral temporomandibular ankylosis and the difficulties associated with its treatment.
Observation: A 16-year-old female patient from a neighboring country was admitted to the UTH- mother-child “Luxembourg” of Bamako for dental pain with facial deformity. Clinical and radiographic examination revealed the diagnosis of Topazian type III bilateral temporomandibular ankylosis.The surgery was performed under general anesthesia, resulting in resection of the ankylosis blocks with bilateral coronoidectomy followed by interposition of the temporal muscle. The per operative oral opening was 45 mm. A resin intercuspidation wedge was placed at the end of the procedure to maintain the amplitude of the mouth opening. Early postoperative rehabilitation was initiated within 24 hours. The operative sequences were simple. The patient was reviewed 6 months after surgery with a mouth opening at 40 mm.
Conclusion: Temporomandibular ankylosis is a very disabling, unaesthetic condition and its incidence is still significant in some developing countries.
