Association between intensity of smoking and periodontal pockets among young university students

Il est admis que fumer est un facteur de risque de progression de la maladie parodontale.
Objectifs : L'objectif de cette étude était d'évaluer la relation entre le degré et la durée du tabagisme et la présence de poches parodontales chez l'adulte jeune en prenant en considération également leurs habitudes d'hygiène orale.
Matériels et méthodes : Il s'agissait d'une étude comparative réalisée à partir d'un échantillon choisi de 357 étudiants jordaniens âgés de 18 à 28 ans. Les sujets ont été interrogés sur leur consommation : nombre de cigarettes par jour et durée du tabagisme. L'examen clinique a été réalisé par un parodontologiste expérimenté à la détection des poches parodontales (profondeur des poches ³ 5 mm). Les sujets ont été divisés en 4 groupes : Groupe A (fumeur avec poches en divers endroits), groupe B (fumeur sans poche parodontale), groupe C (non-fumeur avec poches) et groupe D (non-fumeur sans poche). Les méthodes d'analyses étaient les statistiques descriptives, le test Khi², l'analyse de variance simple et la régression logistique (p < 0.05).
Résultats : L'étude a montré une prévalence des poches chez les fumeurs (groupe A) contre 11.8% chez les non-fumeurs (groupe C), montrant donc une réelle association entre poches parodontales et tabagisme (p < 0.0001). L'odds ratio du groupe A comparé au groupe C était 5 :1. Plus le sujet fumait beaucoup et depuis longtemps, plus la présence de poches était importante (p < 0.0001). Une association significative de présence ou absence de poches a été notée selon les habitudes d'hygiène buccale et cela dans les 4 groupes (p < 0.05).
Conclusions : Cette étude confirme l'association significative entre tabagisme et présence de poches parodontales chez les fumeurs jeunes, particulièrement liée à la durée et au degré de tabagisme.
Smoking is known to be a risk factor for the progression of periodontal disease.
Purpose: The aim of this study was to examine the association between intensity and duration of cigarette smoking with the presence of periodontal pockets at a young adult age in relation to oral hygiene practices.
Materials and Methods: A comparative study was carried out on a systematically selected random sample of 357 Jordanian university students aged between 18-28 years. Subjects were interviewed about their smoking habits for the number of cigarettes smoked per day and the duration. Clinical examination was performed by a qualified periodontist for the detection of periodontal pockets (probing depth ³ 5 mm). Subjects were divided into four groups: Group A (smoker with pockets at many sites), Group B (smoker with no pockets), Group C (non-smoker with pockets), and group D (non-smoker with no pockets). Data analysis included descriptive statistics, Chi-square test of association, One-Way ANOVA, and Logistic Regression were performed (P < 0.05).
Results: The study showed 40.2 % pockets prevalence among smokers (group A), while it was only 11.8 % for non-smokers (group C), indicating strong association between periodontal pockets and smoking (P < 0.0001). Odds ratio for group A compared to group C was 5:1. In addition, the heavier the dose and the longer the duration of smoking, the more periodontal pockets were present (P < 0.0001). A significant association of the presence/absence of periodontal pockets and patient’s own oral hygiene practices was found among the four groups (p < 0.05).
Conclusions: Among young smokers, this study has confirmed existence of significant association between smoking and presence of periodontal pockets, especially with increased intensity and longer duration of smoking.

The need for orthodontic treatment in a school and referred population of Nigeria using the Index of Orthodontic Treatment Need (IOTN)

Objectifs : Déterminer le besoin objectif de traitement orthodontique d'un groupe d'enfants scolarisés et d'une population reçue en consultation, ceci en utilisant l'Indice de Besoins pour le Traitement d'Orthodontie afin d'établir une relation entre le besoin subjectif et objectif de traitement orthodontique.
Patients et méthodes : Le groupe d'étude était constitué de 250 enfants scolarisés et de 157 enfants reçus en consultation au service d'orthodontie du C.H.U Obafemi Awolowo. Ils ont évalué eux-mêmes leur besoin de traitement. Leur moulage ont été également évalué par l'examinateur (KAK) en utilisant l'indice de besoins pour le traitement d'orthodontie.
Résultats : L'évaluation professionnelle du besoin en traitement orthodontique chez les enfants scolarisés montrait les résultats suivants : 62.8 % n'avaient aucun besoin, 30 % un besoin modéré et 7.2 % nécessitaient un grand besoin de traitement orthodontique. La population issue de la consultation représentait respectivement 19.7%, 36.3 % et 43.9 %. En ce qui concerne la santé bucco-dentaire des enfants scolarisés, 66 % n'avaient aucun besoin, 20% un besoin modéré et 14% un réel besoin. Ces pourcentages étaient pour la population venue en consultation, respectivement de 20.4 %, 16.6 % et 63 %. Des différences statistiquement significatives étaient retrouvées dans les deux populations entre l'évaluation subjective du patient et celle du professionnel dentaire. Il n'y avait pas de différence significative en fonction du sexe.
Conclusion : Les résultats de cette étude indiquent que le besoin en orthodontie est évalué différemment par le patient et le professionnel de santé bucco-dentaire. Les traits occlusifs déviants mentionnés par l'indice de besoins en traitement d'orthodontie sont fiables et peuvent être utilisés pour justifier un besoin de traitement.
Objectives: To determine the objective orthodontic treatment need of a group of school children and a referred population using the Index of Orthodontic Treatment Need (IOTN) and establish the relationship between subjective and objective orthodontic treatment need.
Participants and methods: The study groups were 250 school children and 157 children referred to the orthodontic unit of the Obafemi Awolowo University Teaching Hospital Complex (OAUTHC). They assessed their need for treatment, their dental casts were also assessed by the examiner (K.A.K) using the IOTN.
Results: The professional assessment of treatment need of the children in the school population based on the Aesthetic Component of IOTN were 62.8% no need, 30% moderate need and 7.2% great need for orthodontic treatment, the referred population had 19.7%, 36.3% and 43.9% respectively. The Dental health component resulted in 66% no need, 20% moderate need and 14% great need for treatment in the school population. These percentages were 20.4%, 16.6% and 63% respectively in the referred population. Statistically significant differences were found between subjective and professional assessment of orthodontic treatment need in both populations. There were no significant gender differences in objective orthodontic treatment need.
Conclusion: The results of the study indicate that there is a difference of opinion on orthodontic treatment need between laypersons and professionals. The IOTN reliably records deviant occlusal traits can be used to prioritize treatment need.

Relations entre performance masticatrice et état nutritionnel chez des sujets porteurs de prothèses complètes

Pour de nombreux auteurs, les dysfonctions masticatoires peuvent entraîner une réduction des ingestats de plusieurs nutriments essentiels avec comme conséquence possible une malnutrition par carence. Le but de ce travail est d’étudier l’association entre la performance masticatrice et l’état nutritionnel chez des sujets porteurs de prothèses complètes.
Méthode : La performance masticatoire a été évaluée par une méthode granulométrique sur une population de 74 sujets répartis en 2 groupes selon leur état dentaire. Le mini nutritional assessment a été utilisé pour l’évaluation de la nutrition.
Résultats : Les résultats montrent une association entre le port de prothèses complètes et un mauvais état nutritionnel. Le risque de malnutrition évalué par le MNA est trouvé chez 80% des sujets édentés porteurs de prothèses et chez seulement 26 % des contrôles normodentés de même âge. La différence entre les 2 groupes au regard de ce paramètre est significative (p < 0,0001). Il existe une corrélation positive et significative entre la performance masticatoire évaluée par la médiane de la taille des particules et l’état nutritionnel.
Conclusion : Aucun des sujets inclus dans cette étude n’est réellement en état de malnutrition selon le MNA, toutefois une majorité des sujets porteurs de prothèses complètes sont en risque de malnutrition.
For many authors masticatory dysfunctions are related to reduction in dietary intake with malnutrition as a possible consequence. The aim of the present study was to further investigate the relationships between masticatory performance and nutritional state in complete denture wearers.
Method: Masticatory performance was assessed using a granulometric method in 74 subjects divided into 2 subgroups according to their dental status (normal dentition and complete dental wearers). Nutritional status of study subjects was analyzed with Mini Nutritional Assessment (MNA).
Results: There were a significant association between wearing denture and Nutritional Status as assessed by MNA. Using this scale, a risk of malnutrition was found in 80% of subjects wearing complete denture and in 26% in the normal dentition sub group (p < 0.0001). There were also a positive and significant correlation between, the particle size distribution and masticated test food and nutritional status.
Conclusion: Complete denture wearers are at greater risk of malnutrition than did subjects with normal dentition.

Socio-economic status and utilisation of orthodontic services in a Nigerian hospital

Objectif : Etudier l'influence du statut socio-économique sur l'utilisation des services d'orthodontie et la prise de décision du traitement orthodontique à l'Hôpital Universitaire Nigérian.
Matériels et méthodes : Les informations utiles à l'étude ont été enregistrées à partir du dossier médical du patient. Un total de 157 patients vus entre décembre 2002 et décembre 2004 furent enregistrés. Les données sociodémographiques de chaque patient ainsi que les autres informations cliniques nécessaires ont été récupérées dans le dossier médical. Ensuite, les patients ont été classés en fonction de leur statut socio-économique en utilisant le système de classification standard des métiers (12). La classe sociale I représentait ceux qui avaient les meilleurs revenus tandis que la classe sociale IV représentait ceux qui avaient les revenus les plus faibles.
Résultats : Des 157 patients vus pendant la période de l'étude, 86 (54.8 %) faisaient partie de la classe sociale I, 42 (26.7%) de la classe sociale II, 22 (14 %) de la classe sociale III et 7 (4.5 %) de la classe sociale IV. Soixante-trois pour cent des patients étaient de la classe squelettique I, 23.5 % de la classe squelettique II, alors que 13.5 % des patients étaient de la classe squelettique III. 29.3 % des patients avaient un surplomb incisif normal et 38.8% présentaient un recouvrement antérieur normal. (p < 0.05).
Conclusion : La majorité des patients ayant recours au Service d'Orthodontie provient de classe sociale aisée, la décision d'un traitement orthodontique est principalement lié au coût et non au besoin ou à la demande.
Objective: To study the influence of socio-economic status on the utilization of orthodontic services and the uptake of orthodontic treatment in a Nigerian teaching hospital.
Materials and Method: Relevant information needed for the study had been previously recorded in the patient’s case file. A total of 157 patients that presented from December 2002 to December 2004 were reviewed. The socio-demographic data of each patient and all other necessary clinical information were retrieved from the patients case files using a common data abstraction form. The patients were further categorized according to their socio-economic status using a modification of the standard occupational classification system (12). Social Class I represented those with the highest income while social class IV represented those with the lowest income.
Results : Out of the 157 patients that presented during the period of review, 86(54.8%) were from social class I, 42 (26.7%) from social class II, 22 (14.0%) from social class III and 7 (4.5%) from social class IV. Sixty three percent of the patients presented with skeletal Class I, 23.5% presented with Skeletal Class II, while 13.5% presented with Skeletal Class III, 29.3% of the patients presented with normal over jet and 38.8% patients presented with normal overbite. The Social Class of the patients had a significant effect on the skeletal pattern and overbite (p<0.05).
Conclusion: Majority of patients utilizing Orthodontic services are from the higher social classes, therefore the uptake of orthodontic treatment is mainly a function of cost and not need or demand.

Discriminatory attitudes towards patients with HIV/AIDS by dental professionals in Nigeria

Buts et objectifs : L’étude a cherché à étudier s’il existe des pratiques discriminatoires envers des patients atteints par le HIV/SIDA parmi les professionnels de la santé dentaire au Nigéria, comment les professionnels et les établissements dentaires réagissent lorsqu’il faut soigner un sidéen, et quels sont facteurs fondamentaux qui peuvent contribuer aux pratiques discriminatoires.
Matériels et méthodes : Etude transversale conduite dans un état choisi de façon aléatoire dans chacune des six zones géopolitiques du Nigéria. La collecte de données a été faite par le biais d’un questionnaire auto-administré. Il portait sur des informations démographiques, sur la connaissance générale du HIV/SIDA, sur le contrôle de la source d’infection, sur la crainte d’exposition professionnelle, sur les attitudes face à des patients HIV positifs et sur les besoins de formation. 75 professionnels de santé dentaire ont participé à l’étude.
Résultats : Les résultats de l’étude suggèrent qu’un nombre significatif de professionnels de santé dentaire au Nigéria ont des attitudes et un comportement discriminatoires envers les patients HIV positifs.
Conclusion : Davantage d’efforts devraient être faits pour réduire et supprimer la discrimination envers les personnes atteintes du sida afin que celles-ci accèdent à des soins dentaires de qualité.
Aims and objectives: The study sought to investigate whether there are discriminatory practices towards HIV/AIDS patients among dental professionals in Nigeria, how receptive are dental professionals and institutions to treating People Living With Aids (PLWA), and what underlying factors if any that may contribute to any discriminatory practices.
Methods and materials: It was a cross-sectional study conducted in one randomly selected State in each of the six geo-political zones of Nigeria. Data collection was by self administered questionnaire. The questionnaire asked about demographic information, general knowledge of HIV/AIDS, adequacy of infection control, fear of occupational exposure, attitudes towards HIV positive patients and areas of need for further training. A total of 75 dental professionals participated in the study.
Results: The results of the study suggest that significant number of dental professionals in Nigeria reported attitudes and behaviour towards HIV patients which are discriminatory.
Conclusion: Further efforts should be made to promote accessibility of discrimination-free quality oral and dental care to this population.
