Introduction : L’Hémorragie Digestive liée à l’Hypertension Portale (HDHTP), est une urgence médicale dont la Qualité de la Prise en Charge (QPC), qui conditionne la survie du patient, dépend de l’existence d’un plateau technique performant et d’un personnel médical qualifié. Le but de cette étude, a été d’évaluer la QPC des patients admis au CHU de Yopougon, pour HDHTP selon un score de qualité préétabli.
Matériels et méthode : Quarante-sept patients (âge médian 43 ans, extrêmes 20 et 72 ans, 15 femmes et 32 hommes), admis aux urgences et dans le service d’hépato-gastro-entérologie pour une HDHTP, ont été rétrospectivement inclus. Score de Rockall ≤ 2 : 2 patients, entre 3 et 7 : 27 patients et ≥ 8 : 18 patients. La QPC a été déterminée sur une échelle de onze variables, résumant les gestes à effectuer en cas HDHTP, selon les recommandations de la conférence européenne de consensus sur l’HTP (QPC «excellente» : score ≥ 8, «bonne» : score compris entre 6 et 7, «mauvaise» : score inférieur ≤ 5, sur un score total de 15 points).
Résultats : Tous les patients ont eu à l’admission, une prise de voie veineuse et reçu des solutés de remplissage vasculaire (cristalloïdes ou macromolécules), une NFS, un groupe sanguin. Le délai (médian + extrême), d’admission à l’hôpital et de réalisation de la gastroscopie, a été respectivement de 24 (3-720) heures et 24 (6-240) heures. Un tamponnement œsophagien, une ligature et une sclérose des Varices Œsophagiennes (VO), ont été réalisés respectivement chez 3 (6,4 %), 5 (10,6 %) et 2 (4,3 %) patients. Aucun patient n’a eu de drogues vaso-actives pendant l’épisode hémorragique. Globalement la QPC de l’HDHTP a été jugée mauvaise sur l’ensemble des patients, (score médian 5, extrêmes 3-10). Parmi les patients à haut risque de re-saignement ou de mortalité, (45 patients avec score de Rockall ≥ 2), seulement 6 (13,3 %) patients ont eu une ligature ou une sclérose des VO.
Conclusion : La QPC de l’HDHTP a été globalement mauvaise, au CHU de Yopougon. L’absence d’un plateau technique efficient ne permet pas la mise en route du traitement spécifique des VO, principale cause d’HDHTP chez la plupart des patients (non utilisation de drogues vaso-actives, absence de kit pour ligature ou sclérose de VO), auquel il faudrait ajouter, le retard d’admission des patients à l’hôpital et l’impossibilité à réaliser une gastroscopie dans les délais requis.
Background: Gastroesophageal variceal bleeding is a major complication of portal hypertension. The management of this type of bleeding depends on the existence of suitable conditions and a qualified medical staff. A efficient Quality of Management (QM), cannot be reached in developing country where suitable conditions of treatment of gastroesophageal variceal bleeding were frequently inexistent. The aim of this study was to assess the QM of patients admitted in Yopougon’s Teaching Hospital for gastroesophageal variceal bleeding according to the different recommendations for the management of gastroesophageal bleeding related to portal hypertension.
Materials and method: Forty seven patients (mean age: 43 years, range: 20 to 72 years, 15 women and 32 men) admitted in the emergency care unit and gastroenterology unit for Gastroesophageal variceal bleeding were included. Rockall’s score under or equal 2: 2 patients, range from 3 to 7: 27 patients with Rockall score up or equal 8: 18 patients. The QM was established according to a scale of eleven variables to be performed in case of gastroesophageal variceal bleeding related to portal hypertension, based on the recommendations of different consensus conferences and clinical trials on the management of portal hypertension. The QM was considered «excellent»: score ≥ 8, «good»: score range from 6 to 7, and «inefficient»: score under or equal 5, total score of 15 points.
Results: All patients have had in emergency, a perfusion of crystalloids or macromolecules solution, full blood count and blood group. Time interval (median + extremes) for entry in hospital and performing gastroscopy were respective-ly: 24 (3-720) hours and 24 (6-240) hours. Balloon tamponnade, endoscopic variceal band ligation or sclerotherapy were performed in 3 (6.4%), 5 (10.6%) and 2 (4.3%) patients. No patient had vaso-active drugs. Globally the QM of patient with gastroesophageal variceal bleeding was considered “inefficient”: median score 5, (extremes 3-10). Among patients at high risk of re-bleeding or death (45 patients whose Rockall’s score was up to 2), only 6 (13.3 %) of them have had endoscopic variceal band ligation or sclerotherapy.
Conclusion: The QM of gastroesophageal variceal bleeding causes by portal hypertension was globally “inefficient” in Yopougon Teaching’s Hospital. The main cause was the lack of suitable conditions of management (no use of vaso-active drugs, lack of endoscopic variceal band ligation or sclerotherapy), in addition, the difficulty to perform a gastroesophageal endoscopy in short time and the long delay of hospital admission of most patients after the onset of gastroesophageal variceal bleeding.
