L’Hypertension Artérielle (HTA), constitue un problème de santé publique. Peu d’études exploitent les complications néphrologiques de l’HTA. Notre objectif était d’établir le profil épidémio-clinique et évolutif des néphropathies hypertensives dans un échantillon de patients hypertendus malgaches.
Matériel et méthode : Il s’agit d’une étude descriptive et analytique d’une cohorte de 132 patients présentant une néphropathie hypertensive, hospitalisés dans le service de néphrologie de l’Hôpital Joseph Raseta Befelatanana, centre hospitalier universitaire Antananarivo de mai 2004 à mai 2008.
Résultats : La néphroangiosclérose était retrouvée chez 20,06% des patients hypertendus. La moyenne d’âge était de 58 ans, le sex-ratio de 1,40. Les patients avaient un bas niveau socio-économique dans 48,5% des cas. L’hypertension artérielle de grade II, était retrouvée chez 56,1% des patients. Soixante onze pour cent des patients présentaient une HTA à haut risque cardio-vasculaire, évoluant en moyenne depuis 10,28 ans. Le nombre de facteurs de risque cardio-vasculaire, était corrélé à la survenue d’atteinte rénale. L’insuffisance rénale sévère et terminale était rencontrée respectivement dans 31,1% et dans 22,7% des cas. Une protéinurie inférieure ou égale à 1g/24h était constatée dans 72,4% des cas. Quatre décès étaient enregistrés.
Conclusion : L’HTA évoluait depuis plus de 10 ans chez nos patients. Le nombre de facteurs de risque cardio-vasculaire était lié à la survenue d’atteinte rénale. Un dépistage associé à une prise en charge précoce et une lutte contre les facteurs de risque cardio-vasculaire permettraient de prévenir ou de ralentir la progression de la néphroangiosclérose chez les patients hypertendus.
Hypertension is a public health burden. Most of available studies concerned stroke, cardiac and coronary artery diseases linked to hypertension. Hypertensive nephropathy is underestimated. The aim of our study was to establish epidemiological, clinical and outcome of hypertensive nephropathy in a sample of Malagasy patients with hypertension.
Material and method: We conducted a descriptive and analytic study about 132 patients with hypertensive nephropathy at the nephrology department of Joseph Raseta Hospital Befelatanana, University Hospital Centre, Antananarivo from March 2004 to March 2008.
Results: Hypertensive nephropathy occurred in 20,06% of patients with hypertension. Mean age was 58 years, sex-ratio was 1,40. Patients had poor socio-economic condition in 48,5%. Mild hypertension was recorded in 56,1%. Seventy one percent of patients had hypertension with high cardio-vascular risk existing for more than 10 years. The number of cardio-vascular risks was linked to the occurrence of renal impairment. Severe and terminal renal failure was registered in respectively 31,1% and 22,7%. Proteinuria was less than 1,01 g per day in 72,4%. Four patients died during the study.
Conclusion: Patients with poor socio-economic condition constituted the most of our patients. Hyper-tension existed for more than 10 years. The number of cardio-vascular risks was related to the occurrence of renal impairment. Screening combined with early management of cardio-vascular risk might prevent or slow down the progression of renal impairment in patients with hypertension.
