Introduction : Le syndrome stylo-hyoïdien est le sous-groupe le plus fréquent de la stylalgie. Son diagnostic repose sur un trépied au sein duquel l’imagerie est capitale. Dans notre contexte, le recours à la radiographie est habituel malgré plusieurs insuffisances que permet de combler le scanner. Nous rapportons un cas de processus styloïde de 65.4 mm longtemps méconnu.
Observation : Un patient de 50 ans présentait une exacerbation récente d’une symptomatologie cervicale vieille de 15 ans, faite d’une dysphagie et d’une sensation de corps étranger. Le tableau clinique avait motivé plusieurs consultations dans un contexte d’angine à répétition. Pendant que l’examen physique était normal, la dernière radiographie cervicale avait évoqué une hypertrophie du processus styloïde droit et une calcification de la portion distale du ligament stylo-hyoïdien gauche. Le scanner cervical avait confirmé la présence d’un processus styloïde droit géant de 65.4 mm. Après une styloïdectomie partielle, le patient était asymptomatique depuis plus de cinq ans.
Conclusion : Même dans un contexte de limitation des moyens, la stylalgie requiert la réalisation d’un scanner cervical, notamment sa reconstruction tridimensionnelle.
Introduction: Stylo-hyoid syndrome is the most frequent subgroup of stylalgy. Its diagnosis is based on a tripod in which imaging is crucial. In our context, the use of X-rays is usual despite several shortcomings that the scanner can fill. We report a case of styloid process of 65.4 mm long unrecognized.
Observation: A 50-year-old patient had a recent exacerbation of a 15-year-old cervical symptomatology of dysphagia and foreign body sensation. The clinical picture had motivated several consultations in a context of repeated angina. While the physical examination was normal, the last cervical X-ray showed hypertrophy of the right styloid process and calcification of the distal portion of the left stylo-hyoid ligament. The cervical CT-scan had confirmed the presence of a giant 65.4 mm styloid process. After a partial styloidectomy, the patient was asymptomatic for more than five years.
Conclusion: Even in a context of limiting means, stylalgia requires the realization of a cervical CT-scan, especially its three-dimensional reconstruction.
