Objectifs : Evaluer la prévalence des tuberculoses ostéo-articulaires et décrire les particularités cliniques, radiographiques et thérapeutiques de la tuberculose du genou en milieu hospitalier au Gabon.
Patients et méthodes : Etude rétrospective de dossiers de patients avec diagnostic de Tuberculose Ostéo-Articulaire (TOA) porté en rhumatologie dans le service de médecine interne du CHU de Libreville entre janvier 2009 et décembre 2014. Pour le diagnostic de tuberculose du genou la preuve histologique du granulome caséeux était obligatoire. Les paramètres étudiés étaient : la présentation clinique, les aspects radiographiques et l’évolution sous traitement anti-tuberculeux.
Résultats : Parmi 6050 dossiers de patients examinés, 50 avaient un diagnostic de TOA, soit une prévalence de 0,8%. L’âge moyen était de 42,8 ans (16-91) et le sex-ratio de 1,2. La TOA concernait le rachis (spondylodiscite) dans 43 cas (88%), les membres et les sacro-iliaques dans 7 cas (14%) dont 3 tumeurs blanches du genou (6%). Ce dernier était le siège d’une ostéoarthrite destructrice (stade IV de Martini) déformante et invalidante dans les 3 cas (100%). Le délai diagnostique était de 4,3 ans en moyenne (2-7 ans). Deux patients sur trois avaient eu recours à des scarifications au genou. Après traitement classique anti-tuberculeux pendant 9 mois associé à la masso-kinésithérapie, l’évolution clinique était favorable dans les 3 cas.
Conclusion : Le genou est de plus en plus rapporté comme seconde atteinte de la TOA dans les travaux de la littérature, derrière le mal de Pott. Les particularités de la tuberculose du genou chez nos patients sont : le caractère déformant, destructeur et invalidant des atteintes du fait principalement de longs retards diagnostiques ; le recours aux scarifications, à déconseiller vivement et l’évolution favorable après 9 mois de traitement anti-tuberculeux associé à la rééducation fonctionnelle du genou dès le diagnostic.
Aims: To evaluate the prevalence of osteo-articular tuberculosis and to describe the clinical, radiographic and therapeutic characteristics of tuberculosis of the knee in patients seen in a Gabonese hospital.
Patients and methods: A retrospective analytical study of patients followed for osteo-arthritis tuberculosis (OAT) in the internal medicine department of the Libreville University Hospital between January 2009 and December 2014 (6 years). To be included the patients were to have a diagnosis of tuberculosis of the knee based on features of a caseous epithelioid granuloma to the histology after percutaneous biopsies. The clinical aspects of the knee and other associated visceral attacks, the biological profile, the tuberculin skin test and radiographies of the knee were the studied parameters. Septic cases of arthritis and those without histological proof of tuberculosis of the knee were not retained.
Results: Among 6050 files of patients examined, 50 had a diagnosis of OAT, a prevalence of 0.8%. The average age was 42.8 years (16-91) and sex ratio of 1.2. The OAT concerned spine (spondylodiscitis) in 43 cases (88%), the members in 7 cases (14%) including 3 cases of white swelling of the knee (6%). This last was the seat of a destroying osteoarthritis (stage IV of Martini) deforming and disabling in the 3 cases (100%). Mean time from onset of symptoms to diagnosis of the knee tuberculosis was 4.3 years (range: 2-7 years). Two patients among the three were unsuccessfully treated by scarifications of the knee. After 9 months of anti-tuberculous classical treatment associated with masso-kinesitherapy, clinical evolution was favorable in the 3 cases.
Conclusion: The knee is increasingly reported as second attack of the OAT in work of the literature, behind the Pott’s disease. Characteristics of the tuberculosis of the knee among our patients are: deforming, destroying and disabling attacks mainly due to long diagnostic delays; the use of scarification strongly inadvisable and favorable evolution after 9 months of anti-tuberculous treatment associated with physiotherapy of the knee.
