Résumé

La section traumatique de l’artère carotide commune est rare et grave avec un pronostic sombre dû à une mortalité élevée. Elle demeure l’un des problèmes urgents en chirurgie vasculaire et sa prise en charge reste un challenge en situation précaire. Notre objectif était de rapporter un nouveau cas afin de préciser les particularités thérapeutique et pronostique de cette lésion gravissime. Il s’agissait d’un garçon de 11 ans qui avait un traumatisme ouvert cervico-mentonnier latéral droit suite à une chute contre un objet tranchant. Le délai de prise en charge spécialisée était de 2 heures. A l’hôpital, les gestes d’hémostase provisoire étaient inappropriés. L’état hémodynamique du patient devenait rapidement instable. L’exploration chirurgicale immédiate révélait une section complète de l’artère carotide commune droite. La réparation consistait en une anastomose termino-terminale sans interposition de greffon. Le patient comateux en postopératoire immédiat, décédait 24 heures plus tard suite à un infarctus cérébral et une défaillance multi-viscérale. En situation précaire, le pronostic après une section complète de l’artère carotide commune est réservé malgré les efforts techniques hospitaliers apportés par la réanimation médico-chirurgicale.

Summary
A case of traumatic transection of the right common carotid artery in children: contribution to surgical management

Transection of the common carotid artery is a rare event with a poor prognosis and high mortality. It remains one of the most urgent problems of vascular surgery and represents a challenge in the centers without facilities. This work aimed to report a case of patient who presented penetrating cervical injury with isolated total transection of the right common carotid artery, with an emphasis on its therapeutic and prognostic features. It was about an 11-year-old boy who had a right sided penetrating cervical injury after a fall against a sharp object. Time allowed between injury and beginning of the adequate hospital care was 2 hours. At admission, the patient was hemodynamically unstable and underwent an aggressive provisional hemostasis. Surgical exploration revealed a total transection of the right common carotid artery. An end-to-end anastomosis was undertaken without any temporary shunting. After surgery, the patient became comatose and expired 24 hours later as a result of cerebral ischemia and multi-organ failure. In the precarious conditions, prognosis after a complete transection of the common carotid artery is reserved despite the hospital technique efforts provided with medical resuscitation and vascular surgery repair.
