Les auteurs rapportent 4 observations de transsection du grêle au cours de contusion de l’abdomen chez des patients de sexe masculin, dont l’âge variait entre 19 et 27 ans. Il s’agit de lésions rarement observées au cours des contusions, dont la survenue nécessite un traumatisme violent de l’abdomen. Le mécanisme de survenue de ces lésions pourrait être, soit une décélération brutale, entraînant un arrachement, soit un cisaillement ou une compression du jéjuno-iléon entre l’agent vulnérant qui percute violemment la paroi abdominale en avant et le rachis lombaire en arrière. Les circonstances de survenue du traumatisme abdominal, étaient un accident de la voie publique (n = 1), un accident de sport (n = 2) et un accident de travail (n = 1). La laparotomie pour un syndrome péritonéal (n = 3) ou hémopéritoine (n=1) a permis de faire le diagnostic lésionnel. Le traitement a été simple : une résection anastomose (n = 1), un avivement puis anastomose (n = 2) et une iléostomie bitubulaire (n = 1). Le décès observé était dû aux difficultés de prise en charge de l’iléostomie.
The authors report four observations of small bowel transsection after blunt abdominal trauma occurring in male patient which age range between 19 to 27 years. Small bowel injuries are uncommon after blunt abdominal trauma and are usually due to high-energy injury. The blunt abdominal trauma was in relation to motor vehicle accident (n = 1), accident in the work place (n = 1) and a kick in the abdomen by the knee of an opponent during a football game (n = 2). The mechanism of injury is usually mesenteric laceration due to direct compression, or small bowel rupture due to deceleration injury or application of a localised blunt force to the central abdomen, which resulted in jejunal transsection by direct compression against the lumbar spine. Three patients displayed signs of peritonism at presentation and the remaining patient displayed sign of hemoperitoneum. A laparotomy bowel injury was isolated and affected proximal jejunum (n = 2) and the mid-small bowel (n = 2). Three patients underwent a limited segmental resection with primary anastomosis and the remaining patient underwent ileostomy. This patient died at the thirty fifth postoperative day owing to difficulty to take in charge the ileostomy and postoperative infection. We must keep in mind the possibility of occult intestinal injury for all patients presenting with blunt abdominal trauma.
