Objectifs : Déterminer les facteurs étiopathogéniques et analyser les résultats du traitement du prolapsus rectal chez l’enfant.
Matériel et méthodes : Il s’agissait d’une étude prospective réalisée de janvier 2002 à décembre 2008 sur une durée de 7 ans. Elle a concerné tous les enfants âgés de 0 à 15 ans présentant un prolapsus rectal traité dans le service de chirurgie pédiatrique de l’hôpital Gabriel Touré. Les invaginations intestinales prolabées n’ont pas été inclues dans notre étude.
Résultats : Nous avons enregistré en 7 ans, 247 enfants présentant un prolapsus rectal. L’âge moyen de découverte a été de 3,4 ± 2,3 ans. Le sex-ratio a été de 1,06. Les troubles du transit ont été retrouvés dans 85% des cas, une anémie dans 28%, une parasitose intestinale dans 21% et une broncho-pneumopathie dans 7,3%. Le prolapsus était muqueux chez 161 malades (65,2%), total chez 86 autres (34,8%). Le traitement a été médical dans 90% des cas et chirurgical dans 10%. Les résultats ont été satisfaisants dans 72,96% après le traitement médical. Le taux de récidive post-opératoire a été 8% (2 cas). La satisfaction post-opératoire a été de 84%.
Conclusion : Bien que le prolapsus rectal soit multifactoriel dans notre milieu, la constipation demeure la cause dominante. Le traitement médical reste de première intention chez l’enfant.
Aims: Determine the etiopathogenic factors and analyse results of the treatment of rectal prolapse in children.
Material and methods: It was about a prospective study done from January 2002 to December 2008 during 7 years. It has concerned all children aged from 0 to 15 years treated for rectal prolapse in the service of paediatric surgery of the teaching hospital Gabriel Touré. Prolapsed intestinal intussusceptions were not included in our study.
Results: We have gathered in 7 years 247 children with a rectal prolapse. The mean age at the time of diagnosis was 3.4 ± 2.3 years. The sex-ratio was 1.06. Transit troubles like constipation and constipation alternated with diarrhoea were found in 85% of cases, anaemia in 28%, intestinal parasites in 21% and broncho-pneumopathy in 7.3%. Prolapse was mucous in 161 patients (65.2%), total in 86 others (34.8%). The treatment was medical in 90% of cases and surgical in 10%. Results were satisfying in 72.96% after medical treatment. The rate of satisfaction after surgery was 84%.
Conclusion: Although rectal prolapse is multifactorial in our environment; the constipation remains the main cause. Medical treatment remains of first intention in children.
