Pendant longtemps, l’épiglottite aiguë est restée une pathologie spécifique du jeune enfant. L’introduction et la généralisation de la vaccination anti-Haemophilus influenzae de type b a permis une diminution drastique des formes pédiatriques. L’incidence chez l’adulte est en revanche en augmentation, avec une forme clinique et une évolution sensiblement différente de celle de l’enfant. Il s’agit d’une pathologie à prédominance masculine, survenant volontiers entre 40 et 50 ans. La symptomatologie est moins typique et moins bruyante que chez l’enfant, rendant l’évocation du diagnostic parfois difficile. Une prise en charge non-invasive est le plus souvent possible, sous une surveillance rapprochée en réanimation. La mortalité est faible. Nous rapportons un cas clinique d’une épiglottite aiguë chez l’adulte.
For a long time, acute epiglottitis remained a specific pathology of young children. The introduction and generalization of the anti-Haemophilus influenzae type b vaccination has allowed a drastic decrease in pediatric forms. The incidence in adults is on the other hand increasing, with a clinical form and a significantly different evolution from that of the child. It is a predominantly male pathology, readily occurring between the ages of 40 and 50. Symptomatology is less typical and less noisy than in children, making the evocation of the diagnosis sometimes difficult. Non-invasive management is as often as possible, under close supervision in resuscitation. Mortality is low. We report a clinical case of acute epiglottitis in adults.
